Proactive Care Team - Care Coordinator

Location: Alresford, Winchester (Rural)

Hours:18 hours per week – Part time (Monday, Tuesday, Wednesday – 6 hours per day)

Salary: £10.71 per hour

We have an exciting part time opportunity to join our Proactive Care Team (PCT) as a Care Coordinator.  This administrative role involves working as an integral part of the GP Surgery; working alongside the PCT Nurse. They will develop joint ways of working with the virtual ward co-ordinators (SHFT) and social care co-ordinators (HCC) to maximise the use of resources available, avoid duplication and share information to ensure a patient receives the best community care and access to services that will support them staying in their home environment.

The succesful candidate should be able to work independently using their own initiative and also possess excellent communcation and IT skills.

New! Liver Care Pathway Community Nurse

£11.78 per hour £23,035.00 PA

An exciting opportunity has arisen for a part time nurse to work in an exciting and innovative new service. Mid Hampshire Healthcare Limited, a GP Federation representing 18 GP practices and with 216,000 patients has designed and developed a new liver care pathway with patients being seen in the community rather than in secondary care.

Patients will be screened using a fibroscan to measure the liver’s Transient Elastography. This is, according to the manufacturers of the equipment being used, the first time that a community approach to this service has been taken anywhere in the world.

The successful candidate does not need to have any previous experience of liver care. They will be fully trained in how to use the equipment. Following a fibroscan, the liver care nurse will give patients lifestyle advice and in a small number of cases some patients may be referred to secondary care. Appointments will be delivered across the mid-Hampshire geographical area from GP practices, so it is essential that the successful candidate has a clean driving licence.

The service will be delivered in the community three days a week. Appointment times will be between 20 and 30 minutes. On the other two days of the week the liver care nurses will work with secondary care and there will be time for education, case finding and administration.
