ROSEMARY FOUNDATION HOSPICE AT HOME

The Rosemary Foundation Hospice at Home is a registered charity, providing end of life care for individuals who have a life limiting condition and support for their carers, in their homes within a 15-mile radius of Petersfield in Hampshire.
We offer care to both the individual and the entire family affected, from referral to death and afterwards. Whatever the condition and however long a life a person may have left, we offer personal care and emotional support to the individual and their immediate family for as long as it takes, whether that is days, weeks or months. We then continue to support those left behind for as long as they need us to.
We are a nurse-led service providing end of life care, and emotional support, a counselling service and practical help together with nursing care that is patient-based and available day and night, seven days a week, without charge.

NAOMI HOUSE AND JACKSPLACE

We provide individualised care and much needed respite and support for the whole family through the good days, difficult days and last days. This care continues for as long as a family needs.
