MHa Communities

MHA Communities provides support and friendship to older people, living in their own homes across Hampshire, to help maintain independence, improve well-being, encourage inclusion and reduce isolation. Our primary aim is to help local older people to live later life well. MHA Communities is part of MHA which has been providing care, accommodation and support services to older people for over 75 years across the country. We are one of the most well-respected care providers in the sector and amongst the largest charities in Britain.

MHA Communities operates across the whole of Hampshire so no matter where you are located we provide services in your area each offering a programme of services that meet the needs of the people in each locality.

ROYAL VOLUNTARY SERVICE
We work successfully alongside hospitals, local authorities and other organisations, providing much-needed support for older people – delivered with warmth and friendliness by our dedicated volunteers.

AGE UK MID HAMPSHIRE
5 Lunch Clubs a week at various venues in Winchester providing a hot balanced meal and social interaction. Transport provided. Chiropody service for those who no longer qualify for this care from Social Services. Help in the Home Scheme providing light domestic cleaning and shopping enabling the frail older persons to remain in their own homes. Social events for the older person in Winchester offering the opportunity to go on outings and holidays. Information and Advice for older people. Befriending, to help reduce loneliness & social isolation.

COST:   Lunch Clubs £3.60 per meal and £1.70 for transport Help in the Home £10 per hour. Plus £50 annual registration fee. £15 for a chiropody session

COMMUNITY INDEPENDENCE TEAM (CIT)
We are a social care team made up of Social Workers,  Occupational Therapists, Care Managers and Support workers.  We work with people for a period of about 12 weeks to support them to regain their independence to remain living at home.  Types of interventions are getting social activities, help with shopping, cleaning, benefits checks, aids and adaptations, smoke alarms etc.  We will work with a person to do confidence building around mobility etc.  We work generally with people over the age of 55 in Hampshire. We don't work with people who have a HCC funded package of care. We take direct referrals from professionals, friends or relatives can call Hantsdirect to refer on 0300 555 1386.

NEIGHBOURCARE
Run by Andover Neighbour Care. Support for individuals upon leaving hospital. Help can be obtained for: Domestic Support Laundry & Cleaning, Shopping, Collecting Picking up - Prescriptions, Pensions etc.. Help with written correspondence, Preparing meals, Meals on Wheels, Transport Services, General wellbeing.

TQ AT HOME
We are TQ at Home based in Romsey,Lymington and Fareham, the service we are providing is a front door service, to stop patients being admitted to hospital, if all they need is a short term POC, if they already have a POC but they might need more visits.
Myself (Michelle Amos)and Chantelle Hole are the Coordinators for Winchester and Andover immediate response team(winter resilience fund).This is to enable GP'S to be able to rapidly access the support needed to care for patients to stay at home.

THE GOOD CARE GROUP
