The Bluebird Care JET Service is a rapid response, emergency care service that works throughout Mid and North Hampshire to ease bed blocking and congestion in local hospitals. The JET (Joint Emergency Team) Service is designed so patients with acute illnesses and conditions can receive care in their own homes, rather than hospital. This will free up vital space for other patients during the COVID-19 outbreak.
How the JET Service Works
Bluebird Care’s dedicated team are on hand to take referrals 7 days a week between 9am & 5pm. Only Band 6 and above clincians from GP practices, SHF Community and MHH Proactive Care Teams can refer patients in to the service. Short-term care packages will be setup within 4 hours of referral. Each patient referred into the service can access the emergency care for up to 14 days. If ongoing care is required after that point, they will need to be referred to Bluebird Care’s home care team or adult services. Types of activity provided
Moving and handling
Assisting patients in/out of bed
Washing and dressing including showering/bathing etc
Monitoring pressure areas
Assist/prompt prescribed medication
Assist/give fluids and nutrition
Monitoring National Early Warning Scores (NEWS)
Accessing the JET Service
Referral forms should be emailed directly to Bluebird Care. The cut-off time for same-day assessments is 4pm. Referrals received after 4pm will be processed the next day. Care will be provided between 8am-8pm Monday to Sunday, with the number of visits dependent on the patient’s needs.
