Mid Hampshire Healthcare (GP Federation) have partnered with Bluebird Care to provide a JET service. The JET service has been commissioned by West Hampshire CCG to relieve pressures on local health services. The JET service puts in place a same day package of care which could last up to 2 weeks if needed to help prevent hospital admissions this winter and looks to care for patients in the comfort of thier own homes. Research shows care in the community can help speed up recovery.

The service spec

  • Service to commence 16th Jan 17 and currently commissioned to finish on 31st Mar 18
  • Covering all Mid Hampshire Healthcare practices
  • 8-8, 7 day a week Care Assistant support service
  • Rapid same day referral to care should the referral be received before 3pm
  • Up to 4 visits per day if required
  • 2 weeks care packages (if a longer package is required a social services referral should be sent simultaneously)
  • Referrals accepted from GPs, Proactive Nurses and SHFT CCTs for admission avoidance purposes only
  • Only emailed referral forms accepted
